Support Coordination in Sunbury

Support Coordination is a service to help NDIS participants put in place a mix of supports to increase their capacity to manage tasks, live more independently, maintain relationships, and be included in their community. In short, we help you to get the most out of your NDIS funding.

Support Coordinators can assist people of all ages to:

  • Connect to Supports – we can recommend and refer you to a variety of supports, from NDIS supports (e.g. Allied Health Services, Meal Delivery, Plan Management, Support Workers, etc.) to Mainstream supports (e.g. Community Services, Advocacy Services, Education and Employment Assistance)
  • Maintain Supports – we can assist you to utilise your supports by helping you schedule appointments, fill out paperwork, submit feedback, and make informed decisions regarding your supports
  • Monitor your NDIS Plan – we can coordinate with the NDIA, LACs, Plan Managers, other support services, and yourself to ensure your NDIS Plan meets your needs, isn’t being over or underspent, and to help you prepare for your NDIS reassessments, check-ins, and implementation meetings
  • Build Capacity – we can help you to understand and to navigate the NDIS, and provide you with information regarding a variety of disability related topics (e.g. what services are available, assistive technologies and research, goal setting, etc.) and disability related concepts (e.g. neurodiversity, strengths-based practice, active support, etc.)

Meetings with a Support Coordinator can occur in clinic as well as off-site and via telehealth.
